At SentraX Forte Limited, we are committed to protecting your personal and health information. SentraX Forte Limited is a digital health platform designed for continuous chronic disease management, connecting patients, caregivers, and healthcare providers.

1. Information We Collect

To provide our services, we collect:

• Personal Information: Name, phone number, email address, age, gender, and an optional profile photo for your medical passport card
• Health Data: Blood pressure, heart rate, weight, vital signs, clinical notes, diagnosis, prescriptions, medication adherence logs, medical passport details (blood group, genotype, allergies, conditions, diagnostic/lab results)
• Maternal & Child Health Data: If you choose to turn on pregnancy/postnatal tracking (optional, and only offered where relevant), your pregnancy stage and due or delivery date
• Account Data: Patient role, caregiver links, doctor assignments, appointment history
• AI Assistant Data: Messages you send to the in-app Health Assistant
• Document Scanner Data: If you use the passport document scanner, a photo of the document you scan (e.g. a lab report, prescription, discharge note, or vaccination card) is sent to our AI processing service to extract passport details for you to review before anything is saved. If you choose to keep the document photo afterward, it is stored only on your own device — it is never uploaded to or stored in our database
• Voice Input: If you use the microphone to talk to the app, your speech is converted to text by your device/browser's own speech recognition — Sentra-X does not record or store audio
• Marketplace Data: If you order a product, your delivery address, phone number, and order details, shared only as needed to fulfil that order
• Payment Data: Card and payment details are collected and processed directly by our licensed payment processor — Sentra-X never receives or stores your full card details
• Device & Usage Data: App activity, login status, emergency alerts triggered, IP address, device type
• Storage: Most data is stored on your device for offline functionality, and synced to our database when online

2. How We Use Your Information

We use your data to:

1. Monitor and manage chronic conditions such as hypertension, diabetes, and cardiovascular disease
2. Generate risk alerts based on vital sign thresholds: Green, Yellow, Orange, Red, Red Flash
3. Notify caregivers and doctors in real-time during emergency health events
4. Provide dashboards and trend analysis for patients and healthcare providers
5. Schedule appointments and send follow-up reminders
6. Improve SentraX Forte Limited services and enable offline-friendly healthcare in low-connectivity areas
7. Support public health goals: reduce hospital admissions and complications

We do NOT use your health data for advertising.

3. Data Sharing and Disclosure

We respect your privacy. Your data may only be shared with:

• Your consent: With linked caregivers and assigned doctors on the platform
• Healthcare providers: For treatment, diagnosis, and emergency escalation
• Bayelsa State emergency services: When you trigger an Emergency SOS, your name, live location, and a caregiver contact number are sent to the Bayelsa State emergency line as part of our pilot partnership with the Bayelsa State Ministry of Health, to support faster emergency response
• Legal authorities: If required by law or for public health reporting, with de-identified data
• Service providers: The specific technical vendors listed below, each bound by their own confidentiality and data-processing terms, and used only for the purpose stated

Some of these providers process data on servers located outside Nigeria. Where this happens, we rely on the provider's own security and data-protection safeguards, and only share the minimum data needed for them to perform their specific function above.

We do not sell your personal or health data to third parties.

4. Data Security

We implement:

• User authentication and role-based access: Patient, Caregiver, Doctor
• Encrypted data storage and transmission
• Offline-first architecture to ensure data safety in low connectivity environments
• Access controls so only authorized users can view patient information

5. Your Rights

As a SentraX Forte Limited user, you have the right to:

• Access and download your health records
• Request correction of inaccurate health data
• Unlink caregivers or doctors from your account
• Request deletion of your account, subject to medical record retention laws
• Opt-out of non-essential notifications

6. Data Retention

Patient health records are retained to support continuous care. You may request deletion at any time, except where we are required to retain data for legal or medical purposes.

7. Children's Privacy

SentraX Forte Limited is intended for use by adults and caregivers. For patients under 18, a parent or guardian must provide consent and manage the account.

8. Changes to This Policy

We may update this Privacy Policy. We will notify users via the app and email when significant changes are made.
